O-L08 Prognostic value of body morphometrics in patients undergoing surgery for hilar cholangiocarcinoma

Abstract Background Hilar cholangiocarcinoma is an aggressive cancer with poor prognosis. Complex pre-operative workup required prior to major surgery that frequently involves extended hepatectomy biliary reconstruction and associated high levels of post-operative morbidity mortality. Tools predict overall disease-specific outcome are better tailor pre-habilitation interventions selection patients for surgery. Here we investigate whether body morphometrics disease-free survival. Methods Consecutive undergoing resection hilar were identified within a prospectively maintained database in single institution. The CoreSlicer web-based app was used calculate at the L3 vertebral level (muscle, visceral subcutaneous fat areas) from portal-phase CT images. Median cut-offs defined patient groups height-normalised morphometric values compared diagnostic subsequent imaging. Multivariate analysis identify relationships between time diagnosis, changes period outcome. Results Body assessed 88 diagnosis. Of these patients, 53 underwent re-staging enabling assessment change during period. Men displayed significantly higher muscle area, lower than women. High area diagnosis independent predictor reduced survival (HR 1.81, 95% CI 1.1-3.3, P = 0.03), whilst loss skeletal 2.90, 1.0-8.8, 0.05). Patients also appear increased risk post-hepatectomy liver failure (PHLF) experience 30-day mortality those without elevated fat.
